Referrals for clients not meeting eligibility requirements for CHP/MAP Services

Application Packet
The Medical Assistance Program (M.A.P.) application packet is available from 8 am –Noon and 1p – 5 pm, Monday through Friday at the CHP, 1413 Avenue G, Bay City, TX 77414. Fill out the application packet and deliver it, along with all of the required documentation (as outlined below), to the M.A.P. office at the Community Health Clinic. You may mail, fax, or hand deliver the application and copies of all of the required documentation to the M.A.P. office. 申请人必须提供所要求的所有信息和文件，并且必须完整填写申请表, signed and dated.

Required Documentation

Processing of Application with Approval or Denial
